The data, compiled from 3,000 queries across five major AI engines including ChatGPT, Claude, and Perplexity, reveals a stark preference for prestige over scale. While HCA Healthcare operates 187 facilities across 20 states, it failed to capture the primary recommendation in a single state. Instead, AI answer engines favor institutions defined by peer-reviewed research output, high rankings in US News & World Report, and established academic affiliations.
Institutional giants like the Mayo Clinic in Minnesota, the Cleveland Clinic in Ohio, and Johns Hopkins in Maryland dominate their respective regions in AI-generated responses. Ronn Torossian, founder of 5W AI Communications, noted that these systems ignore metrics like bed count or marketing spend in favor of signals tied to medical authority. For hospital systems, the findings suggest that physical footprint is no longer a proxy for digital influence. In the era of generative search, academic reputation has become the primary currency for visibility.
