The 2020 accident occurred when Cowell tested an e-bike shipped from England. Upon pulling the throttle, the vehicle performed a wheelie, launching him seven to eight feet into the air. He landed on his back, instantly recognizing the severity of the spinal trauma. Surgeons initially warned him that fixing the damage might be impossible, requiring an 11-hour procedure to insert a metal rod for stabilization.

This injury served as a catalyst for a lifestyle overhaul. Cowell, who once smoked up to 80 cigarettes a day, committed to significant health changes and lost three stone as he navigated grueling physical therapy. Despite this progress, his relationship with cycling remained precarious. In 2022, he suffered a second accident in London, resulting in a broken arm and facial injuries after falling in the rain. That incident prompted a stern rebuke from his son, Eric, who criticized him for failing to wear a helmet.

Beyond the physical toll, these health scares shifted the television personality’s professional priorities. Cowell now frequently credits fatherhood with grounding his outlook, even suggesting he would support Eric should he choose to follow him into television production. While he remains committed to his role on America’s Got Talent, the upcoming surgery underscores the lingering reality of the injuries sustained half a decade ago.
