Through its network of Partnership Health Centers, Integrity Health utilizes RN-led care teams to deliver primary care, behavioral health, and preventive services. The collaboration with Lightbeam allows these teams to pinpoint patient risks before they escalate into high-cost medical events. Data from 658 higher-risk diabetic members showed that those receiving coordinated care outperformed their peers, even with an average clinical risk profile of 3.07 compared to the 2.62 benchmark.
Financial gains were significant, with $1 million saved in inpatient hospital costs and a reduction in emergency department spending from $4.787 million to $4.632 million. Judy Lagana, Chief Clinical Officer at Integrity Health, noted that the platform enables clinicians to focus resources on areas with the greatest potential impact. Ben Stilwell, President of Lightbeam, emphasized that the model proves how actionable insights and coordinated workflows create a scalable approach to value-based care. The organization now aims to apply these data-driven strategies across other patient populations in New Jersey, Pennsylvania, and Florida.
