Tally and Liminal Partner to Automate Specialty Healthcare Billing

With hospital operating margins dipping below 3% and insurance denial rates climbing, Boston-based startup Tally is turning to agentic AI to overhaul the complex, error-prone world of specialty healthcare billing through a new strategic partnership with the Founders' Studio Liminal.

The U.S. revenue cycle management market topped $125 billion in 2024, yet many providers still rely on manual processes while facing a 41% surge in claim denials. Tally, led by healthcare veteran Peter van Riper and technologist Satish Cheema, aims to replace these labor-intensive workflows with an autonomous system that handles everything from insurance verification to final payment posting. Unlike software that merely highlights billing errors, Tally’s platform functions as an end-to-end operator, feeding real-world adjudication data into its proprietary Payer Behavior Graph.

This partnership leverages Liminal’s expertise in building scalable technical infrastructure to accelerate Tally's expansion into specialty medical verticals often ignored by larger, hospital-focused vendors. By automating the full billing cycle, the company seeks to solve the persistent talent gaps and margin compression currently squeezing healthcare operators. As van Riper noted, the strategy moves away from traditional offshore labor models, instead relying on an AI system that learns the specific reimbursement patterns of individual payers with every processed claim.
